The following downloadable forms are offered by the San Diego RCFL —

Consent to Search

Sample form that may be used for written consent to search computers and other electronic data. The San Diego RCFL recommends that consent be given for an unlimited number of days when possible to complete the forensic analysis.

Service Request

All requests for assistance must include a written service request to information about the case and the services requested. This request may be completed at the San Diego RCFL or in advance by downloading this form. Note: Service Requests that include "Secret" or other highly sensitive information may NOT be faxed or e-mailed to the San Diego RCFL and must be hand delivered.

Sample Non-Participating Agency Letter

Non-participating agencies should write a letter on their stationary that briefly explains the nature of the request. The letter must contain an agency head or delegate's signature and accompany the Service Request Form.
